Objective: The presentation of the clinical case of a young woman diagnosed with bilateral pulmonary embolism who associated a malformation of the inferior vena cava
Case presentation: A 32-year-old female with the twin pregnancy miscarrige for whom the therapeutical abortion was performed 4 days prior to the presentation, accuses paroxysmal dyspnea, pain and functional impotence in the left lower limb. Emergency thoracic tomography was performed, and it showed bilateral pulmonary embolism and also an increased caliber of the azygos and hemiazygos veins. Venous Doppler of lower limbs confirms deep vein thrombosis for the left lower limb, Angio CT of the abdomen and pelvis in the following days was performed, it describing the duplication of the inferior vena cava, the accessory one continuing as the hemiazygos vein after diaphragmatic hiatus.
Case particularity: Deep venous thrombosis in the case of impaired blood drainage through an accessory vena cava, which continues as a hemiazygos vein and then azygos.
Conclusions: Associating inferior vena cava malformation under the conditions of a misscarige in a young patient, without other predisposing conditions, may increase the risk of deep venous thrombosis and implicitly of thromboembolic disease.
