Comparison between the control of risk factors for coronary patients in Romania> results from the EuroASPIRE study III and IV

Introduction: Cardiovascular disease is one of the main causes of death worldwide, in spite of increased medical focus on this subject. That is why primary and secondary prevention should target risk factors by following guidelines. The first three EuroASPIRE trials have demonstrated a poor control of risk factors. Aim: The purpose of this study is to compare the results from the EuroASPIRE (EA) III and IV trials in order to assess the control of risk factors (smoking, obesity, blood pressure, lipids and glycemic control) for coronary patients in Romania. Methods: We have conducted a study on 667 patients (521 in EA III and 146 in EA IV) enrolled in EA III between August – December 2006 and in EA IV between May 2012 – April 2013 at Timișoara Institute for Cardiovascular Disease. Consecutive patients, men and women between the age of 18 and 80, with a history of coronary heart disease or recently admitted in the hospital for this, were included in the study. Results: The prevalence of smoking remained almost constant between the two studies, 10.4% (EA III) compared to 11% (EA IV). However, the prevalence of obesity rose from 38.6 % to 46%. The prevalence of hypertension among coronary patients has greatly improved from 75% (2006) to 48% (2013). Blood pressure control improved dramatically, more than half of the patients reaching the target in 2013 (52%) as opposed to 2006 (32,8%), while total cholesterol control also improved in 2013, when 70% of coronary patients reached the goal of under 4.5 mmol/L, compared to 50.2% in 2006. Furthermore, the prevalence of diabetes remained constant around one third of the patients – 38.6% in 2006 to 39% in 2013. Antiplatelate medication was used by 92% of coronary patients in 2013, more than in 2006, while betablocker use and angiotensin enzyme inhibitors remained almost constant (88%) and (71%). Conclusions: All coronary patients should benefit from a multidisciplinary approach in order to control risk factors, but also to avoid major cardiovascular events, especially since diabetes increases mortality two to five times in coronary patients.
