Introduction: The aim of the current study was to assess the correlation of gene polymorphisms of the renin-angiotensin system with conventional risk factors and cardiovascular complications patients with coronary artery disease (CAD). Methods: The case control study included 405 patients with CAD and acute coronary syndrome admitted to the Chișinău „Sfânta Treime“ Municipal Clinical Hospi tal. Mean age was 57.93 ± 0.34 years. The control group consisted of 290 matched persons without CAD (data used for matching were age, gender, residence and professional activity). Polymorphism of reninangio tensin system: angiotensin-converting enzyme (ACE) gene insertion/deletion (I/D) polymorphism and angiotensin II type 1 receptor gene (AGT1R) were identifi ed by amplified polymerase chain reaction and restricted fragment length polymorphism at the Institute of Genetics and Plant Physiology of the Academy of Sciences of Moldova. Results: Considering the spectrum of risk factors and the clinical presentation according to ACE gene polymorphism, it appears that the presence of homozygous DD state is associated with arterial hypertension (AHT) [r = 0.81, p < 0.01], with recurrence of angina symptoms [r = 0.42, p < 0.05] and a significantly higher risk of cardiovascular death [r=0.27,p<0.05]. Estimation of the correlation between clinical determinants and A/C polymorphism of AGTR gene showed the presence of the risk CC genotype is associated with increased frequency of AHT [r = 0.88, p < 0.01] compared with homozygous AA genotype. Analysis of correlation indices certify that between the carrier of the D risk allele of ACE gene and the C risk allele of the gene AGT1R was a moderate positive correlation (r = 0.58, p <0.001). Conclusions: Carrier state of DD genotype and D allele in ACE gene, being positively correlated with the risk CC polymorphic variant of AGTR1 gene, are markers of increased risk for coronary artery disease associated with arterial hypertension, recurrence of angina symptoms and cardiovascular death.
